Liver Cancer: symptoms, tests and treatment

Primary liver cancer (hepatocellular carcinoma) often grows on a background of cirrhosis from hepatitis B, hepatitis C or fatty liver. Secondary deposits from other cancers are also common. Treatment depends on liver function, tumour size and whether the disease has spread.

Warning signs of liver cancer

  • Pain or fullness in the upper right abdomen
  • Yellowing of the eyes or skin
  • Swelling of the belly from fluid (ascites)
  • Loss of appetite and weight loss
  • Easy bruising or confusion in advanced liver disease

These signs often have other causes. If one lasts more than two to three weeks, see a doctor and get it checked.

Who is at higher risk

  • Chronic hepatitis B or C
  • Cirrhosis from any cause
  • Alcohol-related liver disease
  • Non-alcoholic fatty liver disease
  • Aflatoxin exposure in food

How liver cancer is diagnosed

  1. 1

    Ultrasound and AFP blood test

Often the first checks in someone with known liver disease.

  2. 2

    Triple-phase CT or MRI

Shows the typical blood-flow pattern of hepatocellular carcinoma and helps stage the disease.

  3. 3

    Biopsy

Sometimes needed when imaging is unclear, balanced against bleeding risk in cirrhosis.

Treatment options

The right plan depends on the stage, the type of cancer and your overall health. Most patients receive more than one of these.

Surgery or transplant referral

Selected patients with good liver function can have the tumour removed, or be referred for transplant evaluation.

Systemic therapy

Targeted drugs and immunotherapy for advanced disease that cannot be operated.

Supportive care

Managing ascites, jaundice, nutrition and pain while treatment continues.
